Consent to Treatment Form

Over-the-Counter Medications Form

Physical (within last 12 months)

Internet Acceptable Use Policy

Student Permissions Form

Home & School Volunteer form

Reference form (Kinder-8th grade - need two forms)

Copy of Birth Certificate

Copy of most current Immunization records

Recent Achievement Test results

Report card from previous year (indicating promotion)

 

Returning students need the following:
